mc_annuity() examples.simulate_lifetimes() now constructs lifetime
simulations defensively when mortality inputs contain invalid terminal
values.mc_multilife_status() now uses defaults compatible with
simulate_lifetimes(): sim_id,
life_id, Kx, and Tx.
